There's a video of the build up to the first incident on Twitter, and it looks very much like he puts a fair bit of effort into stamping on Zaha.  A bit like the WBA sending off, you can see the effort he puts into his leg movement. Luckily for him and us it didn't connect.

Zaha initiated it all before the ball was played when he gave Mings a massive shove up near the throat as he moved away from him. He should have kept his cool but unfortunately gave him exactly what he wanted but it was clearly a snidey premeditated tactic from Palace which makes stuffing them with 10 men even better.

El Ghazi was out on the touchline, immediately going nowhere. Zaha was away and bearing down on goal without Mings' intervention.

Mings is clearly an intelligent and articulate person, but he's not always the most intelligent footballer.  He lets players get to him far too easily, and when it works even a non-entity like Bamford had a field day and filled his boots.

In the Twitter video it shows how it started before the actual clash.
Mings and zaha are in the Villa half, Mings touches Zaha’s back (barely) and Zaha turns and punches out at Mings’ stomach.
The ball was no where near them at this stage.

Then Zaha sprints forward to rob the ball off SJM who is on the ground, then it kicks off.

Had Mings gone down during that initial melee Zaha would have been sent off, for sure.

I’m not convinced by the ‘stamp’ either. Mings had to step over SJM to try and get to the ball. There is no way that Zaha could have thought Mings was trying to stamp on him, he had his back to Mings so couldn’t have seen it coming.

Zaha had it in for Mings and successfully conned the ref into getting him sent off.

The most embarrassing thing was Mings positioning before he fouled Zaha for the second yellow. He shaped to make him go out towards the touchline line but got it totally wrong and ended up in a mess.

It's his inability to even turn onto his right side that is undoing time and again. Same for that chance in the first minute. He tries to over compensate by being smart and trying a trick or flick on his left.

Because VAR is about spotting offside nose hairs not violent play.

It'll do Tyrone good to not be the main man. To know he's got to fight for his place like everyone else. I don't doubt he'll put up the fight and work on his game either. He's a good lad, but he needs to sort his head out on the pitch sometimes. All the attributes are there.

I've said it before with Tyrone too, but sometimes he goes for the awkward body position (as you say, often using his left side because he won't use his right). He stretches uncomfortably, or dives in on an awkward position. Apart from increasing the chances of him fouling someone or missing a challenge and being left for dead, it also puts my heart in mouth, that he'll do himself a mischief. He's had injuries before and serious ones, and really needs to do something about his body positioning sometimes, and start working on being able to use his right foot to tackle.

Nasty little calculating shit. Do they still have that panel or committee or whatever it was that meted out retrospective judgements on things like this that had gone unnoticed at the time? That's what happened with Dwight Gayle for his dive a couple of seasons back. Maybe the PL don't bother now there's var.

He was fouled ahead of second offence putting him off-balance in dealing with the ball. I'm not saying he's not prone to the odd mistake, but the ref handled the 2nd situation as badly as he did the 1st.
In the 1st the over-rated palace twunt should have been sent off for his foul on McGinn and then his ott reaction to Tyrone (2 offences, but only one was dealt with meekly).

I didn't pick up on that during the game but at least it explains why Ming's lost his composure and the subsequent, attempted stamp.
